It’s a fair question, and a common one. The honest answer is a little more nuanced than a single number on a chart. Size plays a role, but it’s not the whole story. This guide breaks down what counts as a normal prostate size, when growth starts becoming a concern, and — more importantly — which symptoms actually matter more than the number itself.

What’s a Normal Prostate Size?

In a healthy young man, the prostate weighs somewhere around 20 to 25 grams, roughly the size of a walnut. It sits just below the bladder and wraps around the urethra, the tube that carries urine out of the body.

The prostate isn’t static, though. It goes through two growth spurts in a man’s life: one during puberty, and a second one that starts around age 25 and continues slowly for the rest of his life. That second phase is where benign prostatic hyperplasia, or BPH, comes from.

BPH just means the prostate has grown larger than its original size. “Benign” means it’s not cancer. It’s simply extra cell growth, not a tumor, and it doesn’t raise your risk of developing prostate cancer.

That’s why doctors don’t just look at a number. They look at your symptoms, how much those symptoms bother you, and what your exam and tests show together.

Signs an Enlarged Prostate Might Be Causing Real Problems

Pay attention to your body here. These are the symptoms that suggest your prostate is actually pressing on your urinary system:

  • Needing to pee often, especially multiple times a night
  • A sudden, strong urge to go with little warning
  • A weak stream, or one that stops and starts
  • Straining to start or finish peeing

  • Dribbling after you think you’re done

If you’re dealing with a few of these regularly, it’s worth talking to a doctor, regardless of what size you think your prostate might be. You genuinely can’t tell your prostate’s size just by how you feel — only an exam or imaging can confirm that.

How Doctors Actually Measure Prostate Size

You don’t need to guess. Doctors have a few reliable ways to check:

  1. Digital rectal exam (DRE) — your doctor feels the prostate through the rectum to get a rough sense of size and check for anything unusual, like hard spots.
  2. Transrectal ultrasound — gives a more precise measurement of the gland’s volume.
  3. PSA blood test — measures a protein that can rise with a larger prostate, infection, or in rare cases, cancer. A high or rising PSA usually leads to more testing, not an automatic worry.

Doctors weigh all of this together: your symptoms, your exam, your PSA, and sometimes imaging, before deciding what (if anything) needs to happen next.

What Happens If an Enlarged Prostate Goes Untreated?

A prostate that keeps growing and pressing on the urethra without any attention can lead to real complications over time:

  • Urinary retention — being unable to empty your bladder, which is painful and needs urgent care
  • Bladder damage — from the muscle working overtime against constant resistance
  • Bladder stones — from urine sitting in the bladder too long
  • Repeated urinary tract infections
  • Kidney strain — from pressure backing up through the urinary system over time

None of this happens overnight. It tends to build slowly, which is exactly why it’s easy to brush off symptoms for months, or years, before doing anything about it.

What You Can Do Starting Today

You don’t need to wait for things to get bad before making a few smart changes:

  1. Cut back on caffeine and alcohol, especially in the evening — both irritate the bladder.
  2. Ease up on fluids a couple hours before bed, but stay well hydrated during the day.
  3. Keep your bowels regular. Constipation adds pressure on the bladder.
  4. Stay active. Regular movement is linked to fewer BPH symptoms.
  5. Review your medications with your doctor. Some decongestants and antihistamines can make urinary symptoms worse.
  6. Track your symptoms for a week. Note how often you go, especially at night, and bring that log to your next appointment.

If lifestyle changes aren’t enough, doctors have effective medical options too — alpha blockers to relax the muscles around the prostate, medications that shrink the gland over months, and minimally invasive or surgical procedures for more significant cases. Your doctor will match the approach to your specific situation.
